The On-Line Version of the Federal Post Card Application (OFPCA) is an alternative to the cardstock Federal Post Card Application (FPCA).

The OFPCA is accepted by all States and Territories except American Samoa and Guam. If you send the OFPCA to American Samoa or Guam, the form will not be processed.

The form is in Adobe Acrobat format and can be filled in on-line, printed out, signed, dated, placed in an envelope with proper postage and then mailed to the appropriate Local Election Official (LEO).

If you wish to print out a blank form, you must then complete the form on a typewriter or print with blue or black ink. Once completed, you must sign and date the form, fold it, place it in an envelope, affix proper postage and mail it to the Local Election Official.

Note: Unlike the postage-paid cardstock FPCA, users of the OFPCA must affix postage on the envelope before mailing the form. The OFPCA is being provided as a convenience and as a time-saving alternative when you cannot get the postage-paid cardstock FPCA through other means and so that you will not miss the opportunity to register or make timely request for a ballot. You are entitled to get the postage-paid FPCA and you may request a copy from a Voting Assistance Officer or the Federal Voting Assistance Program.

When completing the OFPCA, please refer to the 2004-05 Voting Assistance Guide. At present, there are no provisions to allow for submission of the FPCA through the Internet.

 

General Instructions for Completing the On-line Form

  • Set the Acrobat Reader Page Size to 150%.
  • Use the TAB Key to move forward from field to field
    (Shift-TAB will move backwards).
  • The Social Security Number field can handle either the entire 9 digit SSN or just the last four digits. It is a free form field so dashes are allowed, if desired.
  • Use the mouse in section 8. You must choose one of the items.
  • Once completed, print out the form, sign and date it.
  • Fold the form, place it in an envelope, affix proper postage and send it to your Local Election Official as stated in the Voting Assistance Guide.

Reasons Local Election Officials have been unable to process the Federal Post Card Application

  • The form was not completed as requested by the applicant's
    state of residence. See the Voting Assistance Guide (VAG).
  • The Local Election Official could not determine a voter's eligibility to vote because the voting residence address was incomplete, incorrect or illegible.
  • A current mailing address was not provided.
  • The form was not signed and dated.
  • The form was illegible.
    Remember to send a new form in when you change your mailing address!
